Summary
Connecticut providers delivering home care, personal care, and home health services must transition from Sandata Mobile to the new Sandata Mobile Connect (SMC) 2.0 application for Electronic Visit Verification (EVV) by September 3, 2024. The old Sandata Mobile app will be completely unavailable after 8:00 AM EST on that date.
Action Required
By September 3, 2024 at 8:00 AM EST: Home care and home health providers must download and transition all staff to Sandata Mobile Connect (SMC) 2.0 application for EVV visit documentation. Develop transition plan immediately, train staff on new app, and review SMC resources on Sandata on Demand. Failure to transition will prevent EVV documentation and could impact billing compliance.
