DSS Important Message - ACTION REQUIRED to receive 2% rate increase through value-based payments (VBP)

By October 15, 2024: Complete all required training modules and learning collaboratives. Returning providers must complete requirements 1-4 (Racial Health Equity training for 25% of staff, HIE Learning Collaborative, Person Centered Planning Learning Collaborative, and Racial Health Equity Learning Collaborative). New providers must also complete requirements 5-7 (sign SDSOA agreement with Connie HIE, provide two HIE administrator contacts, and complete additional trauma-informed care training modules for 25% of staff). Provide agency Tax ID for payment verification. Failure to complete requirements by deadline will result in loss of 2% rate increase payment.
