Summary
Effective July 17, 2026, Anthem will implement validation edits on professional claims to identify unusually high billed charges or units that may indicate data entry errors. Claims exceeding established thresholds will be rejected prior to adjudication with specific error codes (A7:21, A7:583, A7:476). Providers must review rejected claims, correct any errors in charges or units, and resubmit.
Action Required
By July 17, 2026: Billing team must implement validation procedures to review billed charges (SV102) and units/quantity (SV103, SV104) on all professional claims before submission to Anthem. Update billing software and/or establish a pre-submission review checklist to flag unusually high values for individual line items on a single date of service. When claims are rejected with error message 'Charges or Units exceed the standard values for the billed services' or response codes A7:21, A7:583, or A7:476, billing staff must review the original claim, correct any data entry errors in charge amounts or unit quantities, verify accuracy against services provided, and resubmit. Train all billing staff on the new validation thresholds and rejection process. Do not resubmit claims without correcting errors, as repeated rejections will delay claims processing and payment.
