Summary
Substance Use Disorder (SUD) ambulatory providers with level 1 or level 2 certifications must upload their certification letters and enter effective/end dates through the secure web portal by November 14, 2024. Providers with one-year conditional certifications must upload before submitting any claims for Medicaid clients.
Action Required
By November 14, 2024: SUD ambulatory providers must log into www.ctdssmap.com secure portal, navigate to Demographic Maintenance > Provider Certification, and upload their certification letters with exact effective and end dates for each AVRS ID. Providers with conditional certifications must upload before submitting any Medicaid claims or claims will not process.
