Summary
Starting February 1, 2026, radiology prior authorization requests for HUSKY Health members with limited benefit coverage (Family Planning, Emergency Outpatient Dialysis, and Tuberculosis) must be submitted to CHNCT first instead of directly to EviCore. CHNCT will review for benefit coverage before forwarding to EviCore for medical necessity review.
Action Required
By February 1, 2026: Billing team must identify HUSKY Health members with limited benefit coverage (those with both gray CONNECT cards and LB HUSKY Health cards) and submit their radiology prior authorization requests to CHNCT at fax 203-265-3994 using the HUSKY Health Outpatient Prior Authorization Form instead of submitting directly to EviCore. Continue sending all other HUSKY A, B, C, and D member requests directly to EviCore. Update workflow documentation and train staff on the new two-step authorization process.
