Summary
Connecticut Medicaid has restricted the place of service for calcium edetate (J0600) to only hospital outpatient settings and emergency departments. Claims submitted with any other place of service will be denied, effective retroactively to January 1, 2025.
Action Required
Immediately: Billing team must review all calcium edetate (J0600) claims submitted since January 1, 2025, and resubmit any denied claims with correct place of service codes 19, 22, or 23 only. Update billing system rules to restrict J0600 to these POS codes only. Claims with other POS codes will be denied by Connecticut Medicaid.
