Summary
Connecticut Medicaid providers must notify DSS enrollment and QA BEFORE any ownership changes of 5% or greater occur. Failure to notify prior to the change will result in claims recoupment and denial for the period between the change and notification.
Action Required
Immediately: All providers must notify DSS Enrollment at ctproviderenrollment@gainwelltechnologies.com and Nicole Sinisgalli at Nicole.Sinisgalli@ct.gov BEFORE any ownership changes of 5% or greater occur (board changes, stock sales, practice sales, leadership changes). New ownership requires new provider enrollment application and new NPI (except LTC/ICF-IDD facilities). Previous ownership must provide notarized letter on company letterhead detailing sale and enrollment end date. Failure to notify will result in claims recoupment and denial.
