Summary
Federally Qualified Health Centers (FQHCs) with level 1 or 2 certification must upload their certification letters through the secure web portal by June 30, 2025. Additionally, when billing for substance use disorder services (SUD IOP, SUD PHP, or Ambulatory Withdrawal Management), an SUD diagnosis must be listed in the primary position on claims.
Action Required
By June 30, 2025: FQHC providers must log into www.ctdssmap.com secure web portal, navigate to Demographic Maintenance > Provider Certification, and upload their certification letters with effective and end dates for each AVRS ID. Billing team must ensure SUD diagnosis codes are placed in primary position when billing for SUD IOP, SUD PHP, or Ambulatory Withdrawal Management services.
