Summary
Home and Community Based Service (HCBS) providers and Home Health Agencies must now regularly log into the Connie Portal Dashboard monthly during each performance period to qualify for value-based outcome payments. Failure to maintain regular monthly engagement will result in disqualification from these payments.
Action Required
By August 1, 2025: HCBS providers and Home Health Agencies must establish monthly login schedule for Connie Portal Dashboard and launch the HCBS Program Tool application each month during performance periods. Billing team should set up monthly reminders and tracking system to ensure compliance. Failure to maintain regular engagement will result in disqualification from value-based outcome payments.
