Summary
Connecticut Medicaid has added new diagnosis codes to Table 26 for Medical Nutrition Therapy (MNT) services, effective retroactively to July 1, 2025. Providers must resubmit previously denied MNT claims that were rejected due to diagnosis codes not being on the original list.

Immediately: Billing team must access updated Table 26 via www.ctdssmap.com Fee Schedule Instructions to review new diagnosis codes for MNT services. Resubmit all MNT claims previously denied due to diagnosis codes not being on Table 26 for dates of service July 1, 2025 forward. Ensure MNT procedure codes are billed with primary diagnosis codes from updated Table 26.
