Effective April 1, 2026, Connecticut Medicaid will deny all professional and outpatient hospital claims for Dupixent medication. Providers must obtain Dupixent through the pharmacy benefit instead of billing under unclassified drug codes J3590 or C9399.

Before April 1, 2026: Billing team must stop submitting professional and outpatient hospital claims for Dupixent using codes J3590 or C9399. Work with providers to establish pharmacy benefit ordering process for Dupixent. Update billing system to flag these codes for Dupixent to prevent claim denials with EOB 9012. Administration of Dupixent can still be billed separately by licensed practitioners.
