IMMEDIATE ACTION REQUIRED: Billing team must identify all unprocessed claims previously submitted to Blue Cross and Blue Shield of New Jersey Inc. or Prudential Insurance Company of America. For claims with dates of service within the past 12 months: Resubmit on new claim forms with all appropriate attachments (second opinion, abortion/sterilization/hysterectomy forms as applicable) to UnisysParamax using the appropriate CN number for claim type—deadline is 12 months from date of service. For claims outside the 12-month timely filing window: Resubmit on new claim forms with attachments AND include documentation proving the claim was originally submitted timely to former fiscal agents (e.g., copy of original claim with former fiscal agent date stamp, or correspondence from Blue Cross/Blue Shield or Prudential).
