(2) Document any changes to prior authorization requirements, step therapy protocols, or medical necessity criteria for each drug in your billing system and EMR. (3) Update pharmacy verification workflows to reflect new coverage rules for Botox, Briumvi, Daxxify, Dysport, Eylea/Eylea HD, Hyaluronic Acid Viscosupplements, Luxturna, Myobloc, Niktimvo (NEW), Ranibizumab products, Rituximab products, Susvimo, Vabysmo, and Xeomin. (4) Train prescribers and billing staff on the new Niktimvo policy, including any coverage limitations or authorization processes. (5) Ensure claims submitted on or after April 1, 2025 reflect the revised policy requirements. Claims that do not comply with updated coverage criteria will be denied.
