Summary
CareSource has partnered with Versant Health to manage vision services for Indiana Medicaid members. Policy 1309 regarding Medically Necessary Contact Lenses is being updated and will be effective June 1, 2025. Billing teams should verify coverage criteria and prior authorization requirements for contact lens claims under this new policy.
Action Required
By May 15, 2025: Billing team must obtain and review the complete Policy 1309 - Medically Necessary Contact Lenses from the Versant Health website to identify any changes to coverage criteria, billing codes, or prior authorization requirements. Contact CareSource Provider Services at 1-844-607-2831 (Monday-Friday, 8 a.m. to 8 p.m. ET) to clarify medically necessary contact lens billing procedures effective June 1, 2025. Update billing system rules and encounter forms if prior authorization or documentation requirements change. All contact lens claims for Indiana Medicaid members must comply with the updated policy on or after June 1, 2025, or claims will be denied.
