Action Required
Immediately: Billing team must update all MC pharmacy claim forms to newly revised red dropout ink versions. Ensure all data is typed/computer-generated in black ink using 10 or 12 pitch font (block style preferred). Verify proper alignment using the '+' box in field alignment, eliminate all embedded spaces in data fields, format dates as MMDDYY, remove special characters (hyphens, periods, dollar signs), and submit only original top pages to CN address. Forward only computer-generated or typewritten claims for OCR processing - handwritten claims will be processed manually.
