(2) Stop billing antibiotic therapy, hydration therapy, and pain management therapy supplies under incorrect codes—use only the designated procedure code with attached supply listing and invoice/price list documentation. (3) Submit claim adjustment requests using MMIS Claim Adjustment Request Form FD for all incorrectly billed supplies used for antibiotic therapy, hydration therapy, and pain management therapy that were submitted under wrong codes. (4) For TPN supplies, verify correct procedure codes are being used going forward. Failure to submit adjustments for overpaid claims may result in payment recapture by New Jersey Medicaid.
