For each policy, identify: (1) specific CPT/HCPCS codes affected, (2) new prior authorization requirements, (3) coverage changes, and (4) documentation requirements. Update billing software rules, EMR templates, and provider encounter forms accordingly. Train billing and clinical staff on changes before October 1, 2025. Failure to implement changes may result in claim denials for non-compliance with new coverage or prior authorization rules.
