We identified it
Jun 20, 2026
Summary
New Jersey Medicaid has implemented Optical Character Recognition (OCR) scanning for claim processing, requiring providers to use specific UB form specifications including red dropout ink original forms, black typed/computer-generated text in block font, and proper alignment to avoid claim rejections.
Action Required
Immediately: Billing team must switch to revised UB-92 HCFA original claim forms printed in red dropout ink (no OCR copies). Update all claim preparation to use black ink, block style font with consistent print size, ensure horizontal and vertical alignment, and eliminate handwritten fields, correction fluid, special characters, and internal stamps outside remarks section. Mail claims flat in 9x12 envelopes to new Unisys Corporation address. Claims not meeting OCR specifications will be rejected.
