Summary

Newark General Assistance (GA) program is expanding prior authorization (PA) requirements for pharmaceutical services. Effective July 24, 1995, all solid dosage forms and strengths of specified drugs now require PA, including brand name equivalents. This revision replaces Newsletter Volume 5, No. 43 and adds Zantac 150 mg and 300 mg back to the covered drugs list after prior deletion.

By July 24, 1995: Pharmaceutical service providers must implement prior authorization (PA) requirements for all solid dosage forms and strengths of drugs listed in Newsletter Volume 5, No. 52. Providers must obtain PA from the Newark GA program for all brand name drugs with the same chemical entities, strengths, and routes of administration as those on the list. Update pharmacy systems and workflows to flag and route prescriptions requiring PA before submission. Reference the complete drug list included in this newsletter.
