Summary
Effective December 15, 1996, prior authorization is now required for all Protease Inhibitor drugs and Lotrisone Cream for Newark GA and East Orange GA pharmaceutical services, regardless of dosage form or strength.
Action Required
Before December 15, 1996: Pharmacy billing teams must update systems to require prior authorization for all Protease Inhibitor drug products and Lotrisone Cream for Newark GA and East Orange GA eligible beneficiaries. Contact DFD PA hotline at 1-800-609-0106 for questions. Claims without prior authorization will be subject to denial.
