Due to eligibility system delays, providers must now retain copies of monthly municipal welfare identification letters/cards to request 'good faith' payments when GA claims are denied for eligibility reasons.

Action Required

Action needed
Immediately: Billing team must retain copies of monthly GA identification letters or cards from municipal welfare departments. For denied GA claims due to eligibility issues, submit hard-copy claims with attached municipal welfare eligibility documentation to Unisys Good Faith Claims, P.O. Box 4801, Trenton, NJ 08650. For pharmacy claims, report 'good faith' and the ICN of the denied POS claim in field 18 on MC-6 forms.
