Summary
Garden State Health Plan (GSHP), a New Jersey Medicaid HMO, was sold to Managed Healthcare Systems (MHS/AmeriChoice) in four phases between June 1996 and July 1997. GSHP ceased operations as an HMO effective June 30, 1997. Providers must submit all outstanding GSHP claims by the specified service date cutoffs for each county phase, with proof of timely filing required for claims with service dates prior to the applicable phase transition date. Exception: inpatient hospital services remain GSHP's responsibility if admission occurred before the phase cutoff, even if discharge occurred after.

IMMEDIATE ACTION REQUIRED: Billing team must identify all outstanding Garden State Health Plan claims and submit them according to the following phase-specific deadlines before claims expire: Phase 1 (Camden, Essex, Hudson, Passaic, Union counties): Submit all claims for services prior to June 1, 1996 with proof of timely filing attached. Phase 2 (Bergen, Middlesex, Monmouth counties): Submit all claims for services prior to February 1, 1997 with proof of timely filing. Phase 3 (Mercer county): Submit all claims for services prior to June 1, 1997 with proof of timely filing. Phase 4 (Atlantic, Burlington, Gloucester, Morris, Salem, Somerset, Sussex, Warren counties): Submit all claims for services prior to July 1, 1997 with proof of timely filing. For all phases: Inpatient hospital services with admissions before the phase cutoff date remain GSHP's responsibility even if discharge occurred after the cutoff. Submit all claims using established GSHP billing procedures per N.J.A.C. 10:49-7.3.
