Summary
New Jersey Medicaid and NJ KidCare have changed their prior authorization process for partial care services. Prior authorization requests are now processed by Medicaid District Office staff instead of the previous process. All partial care independent clinic providers must complete a detailed facility questionnaire and submit supporting documentation by July 25, 1999.
Action Required
By July 25, 1999: Providers operating partial care clinics in New Jersey must complete the attached questionnaire (sections I-VII covering facility information, ownership, staff, programs, and clients) and submit it with all requested documentation to the Division of Medical Assistance & Health Services, Office of Beneficiary & Provider Services, P.O. Box 712, Trenton, NJ 08625-0712. A separate completed questionnaire must be submitted for each facility location. Billing and administrative staff should verify completion and ensure submission meets the deadline. Failure to submit may result in delays or denials of prior authorization requests and claims.
