Summary
Effective July 1, 1999, New Jersey Medicaid restructured the prior authorization submission routing for Durable Medical Equipment, Medical Suppliers, Food Supplements, and Prosthetic/Orthotic Services. Providers must now submit FD-354 and FD-357 forms to specific regional Medicaid District Offices based on the beneficiary's county of residence rather than a centralized location.
Action Required
By July 1, 1999: Billing and prior authorization staff must update internal routing procedures and contact lists for submitting FD-354 (Medical Supplies and Equipment) and FD-357 (Prosthetic and Orthotic Services) forms. Create a county-to-MDO reference table and distribute to all staff processing New Jersey Medicaid prior authorizations. Update billing system templates to route requests to the correct MDO based on beneficiary county of residence (Burlington/Mercer to Burlington MDO; Camden/Gloucester/Cumberland to Camden MDO; Atlantic/Cape May/Cumberland to Cumberland MDO; Monmouth to Monmouth MDO; Ocean to Ocean MDO). Verify fax and phone numbers are current before implementation. Failure to submit to the correct MDO will result in delayed or rejected prior authorizations.
