Summary
New Jersey Medicaid, NJ KidCare, and PAAD pharmacy providers must submit Form FD-70 Pharmacy Provider Certification Statement along with a copy of their valid pharmacy permit by December 1, 1999. Failure to submit will result in placement in the basic dispensing fee category without qualifying for increments.
Action Required
By December 1, 1999: Pharmacy providers must complete Form FD-70 Pharmacy Provider Certification Statement, attach a copy of valid pharmacy permit, and mail to Unisys at P.O. Box 4804, Trenton, NJ 08650-4804. All questions on the form must be answered or it will be returned. Failure to submit will result in basic dispensing fee category without incremental qualifications.
