Summary
New Jersey Medicaid requires all participating pharmacies to complete and submit a Prospective Drug Utilization Review (PDUR) Certification Statement by December 1, 1999, documenting compliance with OBRA requirements to offer patient consultation on significant medication-related matters. This is a state-mandated compliance reporting requirement for the October 1, 1998 to September 30, 1999 service period.
Action Required
By December 1, 1999: Pharmacy providers in New Jersey must complete the PDUR Certification Statement form and submit it to the NJ Division of Medical Assistance and Health Services, Chief of Pharmaceutical Services, P.O. Box 712, Mail Code #20, Trenton, NJ 08625-0712. The statement requires the pharmacist-in-charge to certify compliance with OBRA Section 4401 requirements by documenting: (1) percentage of Medicaid beneficiaries accepting consultation, (2) percentage refusing consultation, and (3) percentage for whom consultation was deemed insignificant. Failure to submit may result in non-compliance findings from HCFA and potential program sanctions. Retain the completed certification numerically behind the Blue Tab marked '5' in provider files.
