Summary

New Jersey Medicaid, NJ KidCare, and WFNJ/GA require dentists and independent clinics to submit renewed general anesthesia permits from the New Jersey State Board of Dentistry to Unisys (the Division Fiscal Agent) by February 3, 2000, via certified mail with return receipt. Claims for general anesthesia services processed after this date will not be reimbursed until the renewed permit is received and on file.

By February 3, 2000: All dentists and independent clinics providing general anesthesia services must submit a certified copy of their renewed dental anesthesia permit (issued by the New Jersey State Board of Dentistry) via CERTIFIED MAIL -- RETURN RECEIPT REQUESTED to: Provider Enrollment Unit, Unisys, P.O. Box 4804, Trenton, New Jersey 08650-4804. Include your Medicaid/NJ KidCare Provider Number with the permit. Retain the green Certified Mail Return Receipt Card (PS Form 3811) as proof of submission date. Failure to submit by the deadline will result in denial of all general anesthesia claims processed after February 3, 2000, until the permit is received by Unisys.
