Summary
Effective October 15, 2001, Unisys Corporation assumes prior authorization (PA) functions for Mobility Assistance Vehicle (MAV) services in seven additional New Jersey counties (Burlington, Mercer, Morris, Somerset, Sussex, Warren, and Hunterdon). All PA requests for MAV service in these 12 counties must be submitted to Unisys using form MC-12(A), not to the Medical Assistance Customer Centers (MACCs).
Action Required
By October 15, 2001: Billing and prior authorization team must redirect all MAV service PA requests for the seven newly affected New Jersey counties (Burlington, Mercer, Morris, Somerset, Sussex, Warren, and Hunterdon) from MACCs to Unisys Corporation. Update internal routing procedures and staff contact lists to use the new Unisys fax number (609) 588-0816 and mailing address (P.O. Box 4813, Trenton, NJ 08650) for MC-12(A) PA forms and supporting documentation. Notify transportation service providers and any HMOs in your network of the routing change. Failure to submit PAs to the correct entity may result in claim denials or processing delays.
