Summary
Unisys Corporation is assuming prior authorization (PA) functions for Mobility Assistance Vehicle (MAV) and livery transportation services in Hudson and Essex counties, New Jersey, effective January 7, 2002 and February 4, 2002. PA requests must now be submitted directly to Unisys at a new address and fax number instead of through Medical Assistance Customer Centers (MACCs). No changes to reimbursement rates or Division rules.
Action Required
By January 7, 2002: Providers submitting MAV prior authorization requests for Hudson County beneficiaries must redirect all PA requests to Unisys Corporation, Transportation PA Unit, P.O. Box 4813, Trenton, NJ 08650 (FAX: 609-588-0816) using form MC-12(A) with required attachments. By February 4, 2002: Providers in Essex and Hudson counties submitting MAV and livery service PA requests must use the same Unisys address and process. Update internal billing procedures and provider communications to reflect the new PA submission address and contact information. Notify front desk and prior authorization staff of the routing change. Failure to submit to the correct entity may result in delayed or denied prior authorizations.
