Summary
New Jersey's Children's System of Care Initiative (CSOCI) requires CMO (Care Management Organization) providers to submit HCFA 1500 claim forms to Unisys (the fiscal agent) for federal Medicaid reimbursement, effective for services rendered on or after February 1, 2001. Providers must use procedure code Z5008 with applicable ICD-9-CM diagnosis codes and submit claims within one year of service date.
Action Required
Effective immediately for services rendered on or after February 1, 2001: (1) All CMO providers in New Jersey must begin submitting HCFA 1500 claim forms to Unisys instead of relying solely on DHS contract advances. (2) Billing team must use procedure code Z5008 for all CMO care coordination services claims with maximum fee allowance of $850.00 per month. (3) Attach applicable ICD-9-CM diagnosis codes (mental health codes provided in policy attachment) to each claim. (4) Submit all claims within one year of the earliest date of service. (5) Billing team should consider enrolling in Electronic Media Claims (EMC) submission via Unisys for faster processing and reduced errors. (6) Providers must update their 'Pay-To' address in the NJMMIS Provider Master File with Unisys to ensure timely payment delivery. (7) Contact Unisys Provider Services at 1-800-776-6334 for claim form completion assistance or EMC enrollment at 1-609-588-6051. Failure to submit HCFA 1500 forms will prevent the State from claiming federal funding and may delay reimbursement.
