Summary

Effective April 1, 2002, New Jersey Medicaid/NJ Family Care is discontinuing the use of span dating (From Date – Through Date) for occupational therapy, physical therapy, speech-language-hearing therapy, and certain mental health services. Claims submitted with span dates for these services will be denied with Error Code 163. Providers must bill these services using individual service dates only.

By March 31, 2002: Billing team must immediately update billing software and claim submission processes to eliminate span dating for all occupational therapy, physical therapy, speech-language-hearing therapy, and certain mental health services claims. This includes CPT codes H5300, 90870, 92507, 97799 and outpatient revenue codes 420-424, 429-434, 439-444, 449, 912-913 and home health revenue codes 420, 430, 440. Providers must submit individual service dates instead of From Date – Through Date ranges. For all claims with service dates on or after April 1, 2002, ensure each therapy session is billed separately with its own service date. Failure to comply will result in claim denials with Error Code 163. Communicate this change to all clinical staff and providers.
