Summary
Effective October 1, 2002, the State of New Jersey transferred responsibility for care plan reviews for home and community-based waiver cases (ACCAP, TBI, and Model Waivers 1-3) from the Division of Medical Assistance and Health Services (DMAHS) to the Division of Disability Services (DDS). All care plan submissions and inquiries must now be directed to DDS instead of the Medical Assistance Customer Centers (MACC).
Action Required
By September 30, 2002: Billing and case management staff must update all internal contact lists and procedures to route all waiver care plan reviews and inquiries to the Division of Disability Services (Home and Community Services, PO Box 712, Quakerbridge Plaza, Bldg. 11A, Trenton, NJ 08625-0712; Phone: 609-588-2620; Fax: 609-588-7942). Discontinue all submissions to MACC offices. Notify all relevant staff of the change. All current care plan submission procedures remain unchanged; only the receiving agency has changed. Failure to update contact information may result in delayed care plan reviews and processing delays.
