Summary
Effective December 1, 2002, all Personal Care Assistant (PCA) services prior authorization requests and activities must be directed to the Division of Disability Services (DDS) instead of Medical Assistance Customer Centers (MACCs). The same contact addresses, phone and fax numbers remain in use, but the administering department has changed.
Action Required
By December 1, 2002: Billing team must redirect all Personal Care Assistant (PCA) prior authorization requests (FD-365 forms) and related activities to the Division of Disability Services (DDS) instead of Medical Assistance Customer Centers. Update internal procedures to reflect new administrative contact while using the same addresses and phone/fax numbers listed in the policy attachment.
