Summary
New Jersey Partnership for Children is introducing two new Medicaid rehabilitation services - Intensive In-Community and Behavioral Assistance Services - effective January 2004. Providers must complete new Medicaid Rehabilitation Provider enrollment and attend mandatory forums in October 2003 to participate.

By January 2004: Providers interested in offering Intensive In-Community and Behavioral Assistance Services must complete new Medicaid Rehabilitation Provider enrollment application for each county served. Attend one of the October 2003 forums to understand service requirements and billing processes. Submit applications to Partnership for Children, Department of Human Services, PO Box 700, Trenton, NJ 08625-0700. Download required forms from WWW.NJKIDSOC.ORG including service descriptions, Medicaid application, CMS-1500 forms, and attestation forms.
