Summary
New Jersey Medicaid and NJ FamilyCare fee-for-service programs are deleting four local procedure codes (Z1245, Z1250, Z1255, Z1265) effective April 1, 2004. Providers must immediately replace these with equivalent national HCPCS codes (T1021, T1021 with modifier 22, S8990 with modifier 22, and S9129) on all claims with dates of service on or after the effective date. Claims submitted with deleted local codes will be rejected.
Action Required
By March 31, 2004: Billing team must update billing software to discontinue use of local codes Z1245, Z1250, Z1255, and Z1265. Replace all instances with national HCPCS equivalents: Z1245→T1021; Z1250→T1021 with modifier 22; Z1255→S8990 with modifier 22; Z1265→S9129. Audit all pending claims with dates of service on or after April 1, 2004 to ensure compliance before submission. NJ Medicaid and NJ FamilyCare fee-for-service claims submitted with deleted local codes will be denied. Providers should contact Laura Otterbourg at (609) 943-3353 with questions.
