Summary
New Jersey Medicaid and NJ FamilyCare are discontinuing seven local procedure codes (Y4100, Y4200, Y4300, Y4400, Y4610) and the YF modifier effective April 1, 2004. Providers must replace deleted codes with equivalent national CPT/HCPCS codes (V5050, V5014, V5265, V5266) or stop using them entirely where no replacement exists. This affects hearing aid providers submitting claims to NJ Medicaid fee-for-service.
Action Required
By March 31, 2004: Hearing aid providers must immediately update billing software and claim submission processes to discontinue use of local codes Y4100, Y4200, Y4300, Y4400, and Y4610, and modifier YF. Replace with national codes as follows: Y4100→V5050 52, Y4200→V5014, Y4200 YF→V5014 52, Y4300→V5265, Y4300 YF→V5265 52, Y4400→V5266. Code Y4610 has NO REPLACEMENT—do not bill this service. Do not use modifier YF; it has been deleted entirely. Update all claim templates, billing software configurations, and provider encounter forms. Train billing staff on new code mappings. Beginning April 1, 2004, all claims with dates of service on or after that date using old local codes will be rejected by NJ Medicaid and NJ FamilyCare. Failure to comply will result in claim denials and payment delays.
