Summary
New Jersey is deleting the local procedure code Z3370 for Medicaid and NJ FamilyCare fee-for-service claims. Providers must immediately replace Z3370 with the national code H0040 with modifier 22 on all claims with dates of service on or after April 1, 2004. This affects Programs of Assertive Community Treatment (PACT) services billing.
Action Required
By April 1, 2004: Billing team must update all billing systems and claim submission processes to replace deleted local code Z3370 with national HCPCS code H0040 and append modifier 22. Remove Z3370 from all fee schedules, encounter forms, and billing software validation rules for New Jersey Medicaid and NJ FamilyCare claims. Providers submitting PACT services must be notified of this change immediately. Any claims submitted with Z3370 on or after April 1, 2004 will be denied and require resubmission with the correct code.
