Summary
New Jersey updated its Error Code 577 drug utilization review list for Work First New Jersey General Assistance (WFNJ/GA) fee-for-service pharmacy claims. The list now uses specific National Drug Codes (NDCs) instead of drug classes, certain drugs were removed, and prior authorization periods extended from 30 days to up to 6 months. Pharmacists must contact First Health Services for prior authorization on affected medications.

By July 1, 2004: Pharmacy providers and billing staff must obtain the complete Error Code 577 drug list attachment (not fully provided in this document excerpt) and update pharmacy billing system with specific NDCs subject to Errors 577 and 423. Before dispensing any Error Code 577 medications: Contact First Health Services at 1-877-888-2939 for prior authorization; claims will be denied without proper authorization. Update pharmacy protocols to reflect that prior authorizations may now be issued for up to 6 months (previously limited to 30 days) based on medical necessity. The early refill limitation of 90% remains in effect. Note: This policy is from June 2004 and is significantly outdated; verify current DMAHS pharmacy policies with First Health Services before implementation.
