Summary

New Jersey Medicaid is requiring all providers to complete a re-enrollment process conducted by Unisys (the Medicaid Fiscal Agent) starting September 1, 2004. Providers must return completed re-enrollment packages with required credentials and documentation by specified deadlines or face claim denials and program termination. Providers who enrolled on or after June 1, 2003 are exempt from re-enrollment.

By the deadline specified in the re-enrollment package received from Unisys: All providers must complete and return the entire re-enrollment package to Unisys, including the signed provider agreement, updated provider profile, required credentials and documentation, and optional affirmative action survey. Billing team and practice administration must track the re-enrollment deadline from their individualized package cover letter and ensure submission within the required timeframe. Failure to submit a completed re-enrollment package by the specified date will result in pending and eventual denial of all claims and termination of participation in all DMAHS programs.
