Summary
CareSource's third-party payment vendor ECHO experienced a system issue on April 8, 2026, that caused incorrect service dates to appear on Explanation of Provider Payments (EPPs) and 835 Remittance Advice files. The issue affected payment reporting only—not claim adjudication or payment amounts. EPPs were corrected as of April 21, 2026, and ECHO is actively regenerating 835 files. No provider action is required; claims were processed and paid correctly.
Action Required
No immediate action required from billing team. However, billing staff should be aware that: (1) Remittance documents may temporarily display incorrect service dates (01/01/1900 on EPPs, 01/01/2000 on 835s) until corrections are complete; (2) Do NOT resubmit claims based on incorrect service dates; (3) Verify that claims were processed and paid correctly by cross-referencing with claim details; (4) Beginning April 21, 2026, obtain updated EPPs from the ECHO Portal or CareSource Portal; (5) Contact CareSource Provider Services (Medicaid/MyCare Ohio: 1-800-488-0134 or Marketplace: 1-833-230-2101) or ECHO Provider Services (1-888-834-3511) if discrepancies persist or questions arise.
