Summary
New Jersey Medicaid requires pharmacies to annually certify compliance with federal OBRA patient consultation requirements for Prospective Drug Utilization Review (PDUR) matters. Participating pharmacies must complete and submit a certification statement by February 15, 2005, documenting the percentage of Medicaid beneficiaries who accepted consultation, refused consultation, or were deemed not to require consultation.

By February 15, 2005: Pharmacy providers participating in New Jersey Medicaid must complete the PDUR Certification Statement (PDUR-1 form, 10/04) for services provided October 1, 2003 - September 30, 2004. The statement must be signed by a licensed pharmacist (R.Ph.) on behalf of the pharmacy and include: (1) percentage of beneficiaries accepting consultation, (2) percentage refusing consultation, and (3) percentage where PDUR matters were deemed insignificant. Submit the completed form to: State of New Jersey, Department of Human Services, Division of Medical Assistance & Health Services, Chief Pharmaceutical Services, Office of Utilization Management, P.O. Box 712, Mail Code #20, Trenton, NJ 08625-0712, Attention: PDUR. Failure to submit by deadline may result in non-compliance findings and potential program sanctions for participating pharmacies.
