Summary

Action Required
Review enclosed COLA UPDATE REPORT for your facility. If NET INC AFTER amount differs from TAD (MCNH-117) Block #35 PATIENT PAYMENT amount by $1.00 or more: 1) Update Block #35 to reflect actual patient liability amount, 2) Submit TAD to Unisys at P.O. Box 4805, Trenton, NJ 08650-4805 (or update EDI records), 3) Inform CWA or ISS area office that new PR-1 is needed. For differences under $1.00, only update and submit the TAD.
