Effective February 1, 2006, all Adult Day Health Services (ADHS) for Medicaid beneficiaries require prior authorization before services can be provided. Providers must submit prior authorization requests using a specific form to the NJ Department of Health and Senior Services, and services rendered without approved prior authorization will not be reimbursed.

Before February 1, 2006: ADHS providers must implement prior authorization workflow for all Medicaid beneficiaries. Submit prior authorization requests using form PA#1596 to NJ Department of Health and Senior Services for all new patients before initial service provision and annually thereafter. For existing patients enrolled on February 1, 2006, obtain prior authorization before continuing services. Mail completed forms to Adult Day Health Services Unit, PO Box 807, Trenton, NJ 08625-0807. Claims will not be reimbursed without approved prior authorization.
