Immediately: Review the enclosed COLA UPDATE REPORT for all beneficiaries. Compare NET INC AFTER column amounts with current TAD MCNH-117 Block #35 patient payment amounts. If difference is $1.00 or more: (1) Update patient payment amount on TAD form and submit to Unisys, and (2) Notify County Welfare Agency for new PR1 completion. If difference is less than $1.00: (1) Update TAD form only, (2) Do not contact County Welfare Agency. Electronic claim submitters must update patient payment field instead of submitting hard copy forms.
