Summary

New Jersey Medicaid has implemented procedural changes for scheduling non-emergency medical transportation through LogistiCare. Nursing and assisted living facilities must now use LogistiCare's dedicated facility phone line (1-866-527-9945) or fax service to schedule transportation for eligible NJFC/Medicaid clients, and must complete Medical Necessity Forms for each resident requiring transportation.

Immediately: New Jersey facilities must update transportation scheduling procedures to use LogistiCare's facility phone line (1-866-527-9945) or fax service (1-877-457-3316) for all non-emergency Medicaid client transportation. Complete Medical Necessity Forms from LogistiCare's website (http://njfacility.logisticare.com) for each resident requiring transportation. Update staff training on the four authorized service levels: non-emergent ambulance, MAV wheelchair, MAV ambulatory, and livery services.
