Summary
New Jersey is rescinding the November 2009 version of Form FD-70 and requiring pharmacy providers to complete the November 2008 version instead to qualify for optional dispensing fee increments. The deadline has been extended to February 15, 2010.
Action Required
By February 15, 2010: Pharmacy providers must complete and return the revised Form FD-70 (November 2008 version) with a copy of valid pharmacy permit to Unisys at P.O. Box 4804, Trenton, NJ 08650-4804. Failure to submit will result in assignment of basic dispensing fee without optional increments.
