Summary

New Jersey requires hospitals to initiate PASRR (Pre-Admission Screening and Resident Review) screenings as part of discharge planning when patients are being admitted to Medicaid-certified nursing homes. A revised Level I PASRR Screening Tool (LTC-26) must now be used, and training sessions will be coordinated for hospital and nursing home staff.

Immediately: Hospital discharge planning staff must begin initiating completion of PASRR screenings using the revised Level I PASRR Screening Tool (LTC-26) for all patients being discharged to Medicaid-certified nursing homes, regardless of payment source. Download the new form from http://web.doh.state.nj.us/apps2/forms/ and integrate into discharge planning workflow. Attend upcoming DACS training sessions when contacted. Failure to complete pre-admission screening will result in loss of federal financial participation for Medicaid reimbursement of nursing home care.
