Summary

New Jersey medical suppliers who use drop-shipping arrangements for delivering products to Medicaid beneficiaries must now complete and submit a new Contractor/Subcontractor Information Form. This form must be obtained from contractors, certified, and filed with the state Division of Medical Assistance and Health Services.

Immediately: Medical suppliers with drop-shipping contracts must download the new Contractor/Subcontractor Information Form from njmmis.com (Forms & Documents > Provider > Administration). Send form to each contractor/subcontractor for completion. Once returned, certify and mail completed forms to DMAHS Office of Provider Enrollment, P.O. Box 712, Trenton, N.J. 08625. Failure to comply may affect continued participation in NJ Medicaid program.
