Summary

Action Required
By September 1, 2014: Billing team must update all paper UB-04 and 1500 claim forms for New Jersey Medicaid to include ICD version indicators. Enter '9' for ICD-9 codes in Field 21 (1500 forms) or Field 66 (UB-04 forms), and '0' for ICD-10 codes (after October 1, 2015 only). Update billing software and staff training to ensure version indicators match the actual ICD code sets used. Claims will be denied without proper ICD version indicators or if indicators don't match the code sets.
