Action Required

Action needed
Immediately: For patients enrolled in NJ FamilyCare Plan ABP needing long-term care, refer them directly to their Managed Care Organization or local ADRC/AAA for MLTSS program assessment. Stop completing and sending Medically Exempt Attestation Forms to the Medical Assistance Customer Center for these individuals. Update referral processes and staff workflows accordingly.
